HOW WE CAN HELP

Bexley Crossroads respects the individuality of Carers and people with care needs and seeks to promote their choice, independence, dignity and safety.  We also believe that Carers must have access to high quality services that enable them to fully benefit from a break from their caring responsibilities. We aim to provide a flexible and adaptable service that meets the needs of individual Carers and those they care for.

Trained Carer Support Workers go into the home to take over from the carer’s caring responsibilities.  They will undertake the tasks carried out by the carer so long as appropriate training can be accessed. In providing this service they may provide a personal care service for a person with care needs.

This may include support with bathing and feeding, mobility or with the administration of prescribed medication. We may also be able to provide help with more specialist care tasks. Our Personal Care policy provides details of basic and specialist tasks that we are able to undertake – please contact the office for further information. 

PROCESS FOR THE DELIVERY OF CARE AND SUPPORT

CROSSROADS receives referrals from Social Services, The Care Trust, Carers and other voluntary organisationsWhen the referral has been received the Care Administrator will arrange to meet you and the person you care for to discuss the range of services the scheme may be able to provide.  If the Scheme is able to help you, the Care Administrator will agree with you and the person you care for the details of the service, the times when support will be provided and how often this will be.  In order to ensure your safety and the safety of the staff all agreed care tasks would be risk assessed.

All discussions about the provision of the service will be user led and a service plan will be agreed. This will include a personal care plan, which will be developed with, and agreed by the Carer and the person with care needs.  We will ask you both to sign the relevant documents to indicate that you agree with the service to be provided. The Care Administrator or senior staff member will introduce your Care Support Worker to you and the person you care for. You will have an opportunity to explain your expectations of the service at this introduction. The Care Administrator can be contacted at our office if you wish to discuss any issues you may have with the service you are receiving.

The scheme will respond to a referral within 48 hours and arrange a visit no later than 7 working days. Users of the service will be re-assessed annually to ensure that any changes to the provision they are receiving is still appropriate.

KEY TERMS AND CONDITIONS FOR PROVIDING THE SERVICE

The scheme is funded from the Local Authority and Health Partnership as well as external funding.

Service users are to refer to the Client Care Folder for the schemes arrangements for holiday/sickness cover and to when the service may be cancelled or withdrawn.

Carers and the Cared for person will be sent an annual questionnaire to complete to ensure the quality of the service they receive is monitored.

Service Users must contact the scheme in the first instance if they wish to cancel the service. Continuous cancellations without notification will result in your support being reviewed.
